At A&O Devine Care, our Adult Foster Care (AFC) program is designed to support individuals who are unable to live independently due to chronic illness, physical limitations, or mental disabilities. We understand the importance of staying in a familiar environment, which is why our AFC program allows individuals to remain in their own homes, surrounded by family or an approved caregiver, while receiving the care and support they need.

What We Offer:

Personalized Care Plans

Our AFC program is tailored to meet the specific needs of each individual. We work closely with the family and caregivers to create a care plan that maximizes the quality of life for the individual.

Monthly Nursing Visits

Our registered nurses conduct regular monthly visits to monitor the health and well-being of the individual. During these visits, we assess the individual’s medical needs, adjust care plans as necessary, and provide ongoing support and education to the caregiver.

Care Manager Support

In addition to nursing visits, our care managers are actively involved in the AFC program. They provide guidance, resources, and emotional support to both the individual and the caregiver, ensuring that everyone involved feels confident and capable in their roles.
